I have been asked these questions every day by so many international medical graduates 🎓 looking to do US 🇺🇸 clinical experience before applying to the MATCH®. For this reason, we made the #Elective_list 📋 to provide IMGs 👩⚕️👨⚕️ with an updated list 📋 of top-notch institutions 🏥 where they can do electives and all the requirements for each institution including STEP1, STEP 2 CK, visa, fees 💵, insurance, links 🔗, changes during COVID 🦠, etc.
One requirement for all electives is to be a student at the time of doing the elective (you should do the elective before graduation 🎓). Another thing to keep in mind is that processing time ⏳ for electives can take months 📆 in some institutions. Therefore, start looking 👀 at the list and make your plan and schedule so you can complete your elective on time before you finish your medical school.
What are electives in the US?
What are the requirements for doing an elective in the US?
Generally, STEP 1, being a medical student at the time ⏳ of the elective, being in final years of medical school, fees 💴, health insurance, malpractice insurance, and immunization forms. However, the requirements vary by institution, and these are summarized below for each institution.

Although the majority of institutions require STEP 1 for electives, some institutions might not ask for it. You can check 🔍 the requirements for each institution below.

Yes ✅, IMGs can do electives. However, not all institutions accept IMGs. The list 📋 below includes this information.
How much do you pay for electives? Are electives free?
There is a fee 💴 for most electives. The application fee is just to apply and you lose it even if you are not accepted for the elective. The elective fee must be paid after you are accepted. The different fees for different institutions are summarized in the list 📋 below.

Yes ✅. Electives are a great way to get hands-on clinical experience, learn about the US 🇺🇸 healthcare system, and show your clinical experience which can help you get letters of recommendation (LORs) ✉️.
Which is better, electives or observerships?
Electives ✅, as you get hands-on clinical experience during electives while in observerships, you are mainly shadowing someone without doing much 🤷♂️.

What is the VSLO® (Visiting Student Learning Opportunities)?
VSLO – think of it as your passport to clinical rotations at different institutions in the U.S. It’s this super handy online program that lets you find and apply for awesome clinical and research opportunities across the U.S.
In more detail, the VSLO (Visiting Student Learning Opportunities) is an online platform by the Association of American Medical Colleges that connects medical students to diverse clinical and research opportunities across different healthcare systems. This could be a short-term elective, a longer-term research project, or a public health opportunity.
The centralized system streamlines the application process. This allows you to easily apply to multiple institutions without juggling a bunch of different application procedures and deadlines. However, keep in mind that there might still be institution-specific requirements to be mindful of.

Are virtual electives/ telerotations helpful?
Telerotations 💻 are not as helpful as in-person US clinical experience since you won’t be able to demonstrate your clinical skills and ability to understand the team dynamics on virtual platforms. It is also challenging to get a valuable letter of recommendation ✉️ through virtual rotations. Remember that there is a difference between a clinical rotation and a lecture. Virtual rotations are more like school lectures which are totally different from real-life interactions in a hospital.

2. University of California (San Francisco)
Students accepted: final year medical students attending either LCME or COCA accredited schools, or schools with a formal affiliation with JHUSOM. Research experiences are offered on a space available basis to U.S. and internationally trained medical students.
Offer virtual electives, students registered for virtual electives are not permitted to enter School of Medicine research or clinical facilities.
Application: All applications must be submitted through our Slate portal.
Requirements:
- Immunizations: AAMC standardized immunization form.
- Health insurance : has to meet the minimum requirements established by the JHUSOM, travel insurance that meets the J-1 exchange visitor health insurance requirements is an acceptable alternative or enroll in the JHUSOM student health program.
- Visa: B-1/B-2 visa (will be provided with the necessary documentation to support their application).
Fees: registration $500 for each elective + $5500 for visiting students from non-LCME accredited schools + health insurance $330 per month (if applicable)

Students accepted: visiting medical students and international students are accepted. Mayo Clinic’s campus in Jacksonville, Florida, only accepts international medical students from institutions that are licensed by the Florida Commission for Independent Education.
Application: Only through VSLO
Conditions:
- Have passed USMLE Step 1, COMLEX Level 1, or NBDE; international applicants must have passed USMLE Step 1
- Be in the final year of medical school.
- Be in good academic standing at their medical school.
- Have received the required immunizations.
Additional conditions for international students
- Health insurance coverage in the amount of a minimum of $50,000 USD
- If home medical school is not in English, demonstrate fluency in English with a successful TOEFL within the last two years (minimum score 104)
- Have completed approximately 48 weeks of patient care clinical experiences — 6-8 weeks each of medicine, pediatrics, obstetrics and gynecology, and surgery, and approximately 20 weeks of electives.

5. Mayo Clinic College of Medicine and Science
Students accepted: visiting medical students enrolled at US or Puerto Rico LCME accredited or COCA accredited Osteopathic institutions. Currently NOT accepting students enrolled at international institutions with whom they do not have approved agreements.
Application: Only accept through VSLO
Requirements:
- Complete visiting student application
- Proof of professional liability coverage (amount stipulated in the approved affiliation agreement)
- Official transcript from homeschool, indicating successful completion of all required clinical core clerkships in Medicine, Obstetrics/Gynecology, Pediatrics, and Surgery at the time of application.

Students accepted: final year medical students from Ben Gurion University, LCME-accredited school in the US/Canada, Osteopathic school, Sackler School of Medicine New York Program. International students approved in the International Exchange Affiliation agreement.
Currently, all approved international students will apply through OASIS except for approved Ben Gurion University and Sackler School of Medicine New York State Program students, who will apply through VSLO(link is external and opens in a new window). - Must have successfully completed all core clerkships (IM, Obstetrics & Gynecology, Pediatrics, Psychiatry, Surgery, and either Family Medicine or Neurology)
- Good academic standing and approved by the home institution to participate in away rotations.
- English language proficiency (medical school where English is the primary language or TOEFL minimum score 100)

10. Washington University/B-JH/SLCH Consortium
U.S. Clinical Experience

Students accepted: clinical and research experiences are restricted to students from the international medical schools that have affiliation agreements with the School of Medicine.
At this time the Center for Global Health is not establishing new exchange agreements.
Conditions:
- Be in good academic and professional standing
- Passing grade for core clerkships: Medicine, Ob/Gyn, Pediatrics, Surgery, and Psychiatry
- Where English is not language of instruction, provide evidence of proficiency in either of the following ways:
■ TOEFL iBT, taken within the last 2 years, minimum score 100 (Reading 25, Listening 25, Speaking 25 (26 is preferred), Writing 25).
■ Documented passing score on the USMLE Step 2 Clinical Skills Exam.

- Visiting students from a school that has an affiliation agreement with Vanderbilt School of Medicine. Institutions have to sign on to the AAMC Uniform Clinical Training Agreement.

Eligibility Requirements
- Full-time medical students in their final year of study outside the United States, in good academic standing.
- Completion of core clerkships prior to start date of approved UIC elective: Internal Medicine, Obstetrics/Gynaecology, Paediatrics, Psychiatry, Surgery, Family Medicine. If these clerkships are not listed on your transcript at the time you submit your application, the completion dates must be listed by your dean in their letter of good standing.
- TOEFL scores are required if your courses are not taught in English. If your courses are taught in English, your dean must verify this in their letter of good standing.
- Must be fully vaccinated at least 2 weeks prior to elective start date.
Fees: $3200.00 USD (The elective fee is due on the first day of the elective at orientation. We do not allow for earlier payments.)
